Letter of Appointment

Attention: To whom it may concern,

1. We hereby appoint Feldwick Insurance to act as our Insurance Broker until further notice in writing.

2. This Letter of Appointment replaces and revokes any previous authorities given to any agent or broker, with respect to our insurance policies documented below.

3. Please provide Feldwick Insurance with all information they request in respect of our current insurance policies, claims history or other matters in writing. Please also let Feldwick know if we have any insurance policies with you not listed above and provide the details of them to Feldwick Insurance.

4. Please do not cancel any of our current insurance policies unless you received direct instructions from us to the contrary in writing.

5. We acknowledge that you will provide consideration to Feldwick Insurance for placing our insurance with you, and we consent to this.
